Membership Application

Please complete the form below to apply for membership with the Cedar Bend Golf Association.

Once you click the submit button below the form, an email will be sent to the club with your application information and your agreement to the stated terms. You will be contacted to arrange for payment to complete the transaction. We look forward to welcoming you to the club!

    Cedar Bend Golf Association agrees to provide the above named Member unlimited year round golf. Play will only be limited by weather closures, maintenance and dates for Board sanctioned Golf Tournaments

    * Family membership allows husband, wife, and children under 21 years of age, unlimited golf
    ** You must have an annual membership package to qualify for Diamond Membership

    If this agreement is entered into during the interim membership period, the new member will pay the initiation fee and remaining prorated monthly dues. New Business members pay in full for the remaining months.

    This agreement shall start May 1 of the current year and end April 30 of the subsequent year.

    I understand that I am personally obligated to pay the annual dues. If delinquent more than 30 days, I understand that I will be assessed a late fee. If delinquent for more than 60 days, I will lose all playing privileges until past dues and late fees are paid.
